Companion Care

Companion care is primarily emotional support and companionship for seniors who are generally healthy and who want to remain independent at home.

Personal Care

Personal Care Services are provided by nursing aides, personal care providers or home health aides. Through Citadel of Healthcare, our patients can enjoy the way of life they choose while managing a physical condition or illness.

Nursing Care

Our visiting skilled nurses are experts in administering the treatment you need. These programs are provided either by a registered nurse or a certified nursing assistant under the supervision of a registered nurse.

Development Disability Services (DDA)

Respite Care

Respite is short-term care intended to provide both the families and individuals with a break from their daily routines. Respite care relieves families from their daily caregiving responsibilities, while providing the individual with opportunities for new and enriching experiences and facilitates self-determination.

Supported Living

Citadel of Healthcare, Inc. Supported Living services provide participants with a variety of individualized services to support living independently in the community.

Community Development Services

Personal Supports

Our personal support program provides participants with personal assistance in their home. The staff assigned to our participants will teach them appropriate interactions with others at home and in the community. Services include transportation, purchasing, and other supports indicated in the participant’s person centered plan (PCP).

Community Living (Group Homes)

Our residents live in contemporary homes or apartments situated in secure neighborhoods, with no more than three individuals sharing a residence. We actively foster personal growth and independence across various aspects of their lives.
